How to Register

Registration forms are available from any of our sites or you can register online using the form linked below.

We will need some information about you and your health — this speeds up the registration process.

We do not refuse patients within our practice boundary.

Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

If you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.

We have set this practice boundary as visits outside this area can take a clinician out of surgery for too long. This impacts on other patients.

Patients already registered can remain registered, but new patients living outside the boundary are encouraged to find a nearby practice.

Register online

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
