Advice for referrers:

Please note that the following are absolute contraindications:
- Active peptic ulcer, oesophageal varices, aneurysm, and proliferative retinopathy
- Epistaxis and Haematuria
- Recent organ biopsy
- Recent trauma or surgery to the head, orbit, or spine
- Recent stroke
- Confirmed intracranial or intraspinal bleed
- Uncontrolled hypertension >180/110
- Infective endocarditis.
- Pregnancy
- Allergy to Warfarin
- Blood borne Viruses
- Antiphospholipid syndrome
These patients should be referred via haematology to the hospital clinic
And the following are relative contraindications:
- History of gastrointestinal bleeding
- Liver disease
- Renal failure
- Alcoholism
- Mental impairment
- Thrombocytopenia
- Coagulation disorders
- Interacting drugs, in particular non-steroidal anti-inflammatory drugs
- Poor concordance
- Poor attendance for regular blood tests.
These patients may be referred but please inform the provider.
